Primary Care Providers Update

Primary care medical providers (PCMPs) who offer services, "beyond the traditional fee-for-service primary care models of care" can become eligible for recognition and reimbursement for providing enhanced primary care. As per the Department of Health Care Policy and Financing (HCPF) ACC Enhanced Primary Care 9/5/14 document, "...nine factors have been adapted from Colorado Senate Bill 07-130 concerning medical homes for children, national patient-centered medical home models [e.g. National Committee for Quality Assurance (NCQA) 2014], evidence-based practices, RCCO recommendations, and other key Department initiatives designed to incentivize quality improvement. PCMPs who meet five of the nine factors may be eligible for an additional per member per month (PMPM) payment."
