Survey for Oncology Health Care Providers

Dear Colorado Oncology Healthcare Provider,

We are providers from the University of Colorado who have developed a survey to assess oncology health care provider knowledge, attitudes, and behaviors regarding marijuana in cancer patients as part of a research project entitled “Survey for Oncology Health Care Providers.” By completing this survey you will be helping us gather information on this important issue.  The compiled results from oncology health care providers across our state will be used as precursor data for a future nationwide study.  This survey should take less than 15 minutes to complete.
